Step 2-5: Fill in Application Details - Applicant's particulars

1. Personal Certificate: The applicant must be aged 18 or above.
2. Organization Certificate: The company must be a Limited Company and the person who submits the application must be the authorized person of the Limited Company.

Please take the following steps to input the applicant's particulars for Personal Certificate:

1. Select Title of Applicant.
2. Input Surname and Other Name of Applicant, if applicable.
3. Input Local Contact Tel. No.
4. Input Fax No.
5. Input Date of Birth.

On the right bottom of the screen, you can:

• Press <Back> button to go back to the previous screen; or
• Press <Clear> button to clear and re-enter the data; or
• Press <Continue> button to go to the next step to input the correspondence address.

Please take the following steps to input the applicant's particulars for Organization Certificate:

1. Select Type of Limited Company.
2. Select Title of Contact Person.
3. Input Surname and Other Name of Contact Person.
4. Input Local Contact Tel. No.
5. Input Fax No.

On the right bottom of the screen, you can:

• Press <Back> button to go back to the previous screen; or
• Press <Clear> button to clear and re-enter the data; or
• Press <Continue> button to go to the next step to input the correspondence address.
